We collect demographic data from patients to help determine if health disparities exist within the patient populations, including screening for social determinants of health.

We incorporate health equity goals into strategic plans, making health equity a strategic priority.

We participate in activities geared to reduce health disparities across their patient populations.
Quality Improvement
Patient health outcomes are evaluated based on demographic factors.
Leadership Engagement
Senior hospital leadership and the hospital board of trustees annually review health equity goals and status toward those goals.

LGBTQIA+ Community
Members of the LGBTQIA+ Community can access support and information by contacting our Health Equity and Patient Experience Navigator. All calls may remain confidential. The navigator can provide guidance on LGBTQIA+ providers and services that are knowledgeable and equipped to meet the specific healthcare needs of the LGBTQIA+ patients.
