Senior Management of Agility and Rehab in Trauma

The SMART Program has been created as an additional process at Conemaugh Memorial Medical Center's Geriatric Trauma Institute to aid in the recovery of geriatric trama patients. The clinic offers follow-up care for older adult patients who have experienced a fracture.

Evaluation for a risk for falls
Referrals to the appropriate rehab source
Complete medical review
Review of Dexa Scan and medications for osteopenia and osteoporosis
Physical therapy exam that will identify the need for further rehab
Review of pain regimen, urinary incontinence, insomnia, and sleep apnea
Evaluation of equipment need for safe walking
Evaluation for balance training and vertigo
Home evaluation through home health with recommendations
Eye exam, hearing exam, foot and shoe exam
Evaluation of blood pressure for orthostatic hypotension
Psychosocial evaluation

Eligible Patients

Age 65 or older and has experienced a bone fracture
Under 65 years old and has medical conditions that place them at risk for falls
