Although hospice care is centered in the home, it is sometimes necessary for the patient to receive care for a short period of time in a hospital setting. The Conemaugh Regional Hospice team will help arrange this care and will stay involved with the patient and family, resuming care in the home when appropriate.

The team's multi-disciplinary approach includes the experience and perspectives of physicians with specialized training, the patient’s attending physician, nurses, social workers, counselors, home health aides, trained volunteers, and clergy. Nutritionists and speech, occupational, and physical therapists are also available as needed. The team meets regularly to evaluate and coordinate the care plan of each patient.

A patient can be referred to Conemaugh Regional Hospice by family members, physicians, home health nurses, social workers, clergy, friends, or neighbors.

Hospice care is covered under most insurance plans. However, if the insurance plan does not have a Hospice Benefit or if the cost of a patient’s care exceeds what insurance will pay, the patient’s care will not be affected. In these cases, patients may apply through the finance department and demonstrate financial need and if the patient meets the requirements, assistance with meeting costs directly related to the patient’s terminal illness will be provided. Conemaugh Regional Hospice provides care without regard to race, color, age, gender, religion, national origin, handicapping condition, or ability to pay.

Conemaugh Regional Hospice is certified by Medicare and Medicaid, is accredited by The Joint Commission on Accreditation of Health Care Organizations, and is licensed as a Hospice Program in the state of Pennsylvania.

Volunteers Wanted!
