Preparing for Conemaugh Physician Group Visit

Conemaugh Physician Group physicians and staff are dedicated to provided exceptional patient care and creating an ideal patient experience during each visit. 

Here are a few helpful tips to help create a positive experience during your visit.

Items to Bring to Appointment

  • Photo ID
  • Insurance Card
  • Method for payment including cash, check, credit card, or debit card
  • Any completed forms that the physician office sent to you

Write Down Your Questions

Take some time to reflect on current questions or concerns regarding your health prior to your physician visit. Bring a list with you to your appointment and our nurses and physicians will answer your questions and address your concerns. We strive to maintain open lines of communication with our patients and by being prepared for your appointment, we can ensure you are receiving the best possible care.

Medication Information

Bring a list of medications you or your loved one is taking including medication name, dose amount, and how frequently to take the medication. Please include prescribed medications, over-the-counter medications, and supplements.

Arrive Early and Allow for Additional Time

Please allow extra time for traffic, parking, and the check-in process. Delays from your scheduled appointment time may occur as we care for ill patients, and take time to answer questions and concerns of our patients. Please be assured that you will receive the same attention during your visit.

Bring Someone With You and Take Notes

It is often helpful to have someone with you during your appointment to ensure you are asking all of your questions and understanding what your physician or nurse is explaining. Patients are encouraged to take notes and to restate what the physician said to ensure clarity in instructions, medications, and that all questions have been answered.
