Gold Plus Stroke Award - Stroke Care

Conemaugh Memorial reached Stroke Performance Achievement indicators for consecutive 12 month intervals and 75% or higher compliance on at least five of the Get With the Guidelines Stroke Quality Measures to improve quality of patient care and outcomes.

Conemaugh is recognized as an institution with exceptional quality programs that continually strive to improve stroke patient outcomes. Conemaugh, named a Primary Stroke Center in 2010, treats over 700 stroke patients each year providing Stroke Defect-Free Care as measured by the AHA Get With the Guidelines. Members of the Conemaugh Memorial Stroke Team include Nurses of the Neuroscience Service Line, the Department of Emergency Medicine, Intensive Care Unit, Cardiac Care Unit, and nursing care stroke units 10 Rose and E6.

Silver Performance Award - Heart Care

Conemaugh Memorial's journey to silver recognition began in January 2014. Over 1,764 patients have been treated at Conemaugh Memorial for heart failure during this time and heart failure composite performance measures have improved from 50% to 100%. Evidence-based specific beta blockers have improved from 65% to 100% in 2014.

"We are continuously looking for ways to improve our patient’s care and our Congestive Heart Failure outcomes demonstrate that commitment. We have seen a positive improvement in the lives of our patient population and a decrease in the mortality rate from the beginning of our journey," says Kerri Tononi, Executive Director of the Cardiovascular Service Line at Conemaugh Memorial. "As we continue through our journey with The AHA Get With The Guidelines Cardiac Heart Failure program, I am confident that we will continue to see strong results through better patient outcomes and a higher quality of life."

Kelly Macheska, Director of Quality and Systems Improvement, from the American Heart Association presented the awards to members of Conemaugh Memorial's Cardiovascular Service and Neuroscience Service Line leaders.
