Conemaugh Health Foundation has received a $51,560 grant through the Highmark Foundation’s “Addressing Community Health Needs Though Hospital Partnerships” initiative.

The funding will support four comprehensive screenings in 2014 to take place throughout Conemaugh Health System's rural health centers in partnership with the local Alternative Community Resource Program (ACRP). This partnership will allow Conemaugh Health System to expand its existing access to healthcare programs and services to a larger number of the population while providing life-saving outreach to individuals by encouraging healthy behavior through wellness education and physical activities.

“By identifying at-risk individuals for heart disease and diabetes, we are able to provide on-site education of the disease process and information about programs at Conemaugh Health System to help individuals make healthy lifestyle changes for themselves and their families,” says Michelle George, RN, BSN, Project Coordinator of the Conemaugh Cardiovascular/Diabetes Disease Prevention Program. “By partnering with ACRP we expect to increase awareness of the dangers of an unhealthy lifestyle. The Highmark Foundation grant will enable us to reach an even larger underserved and underinsured population throughout our rural communities”.

Community wellness screenings highlight the dangers of high blood pressure, high cholesterol, smoking, obesity and physical inactivity. Participants will be encouraged to meet with Conemaugh Health System experts and discuss personal lifestyle choices and positive health practices.

In a recent Community Needs Assessment for Cambria and Somerset Counties, conducted by Conemaugh Health System and the United Way of the Laurel Highlands, an area for improvement included increased training and education on living a healthy lifestyle. High rates of smoking and obesity were noted as areas of concern in the community.
