Patients who are discharged from the hospital but may require additional care before going home may find that skilled nursing care is the best choice for them. This often includes those recovering from major surgery, stroke, heart attacks, respiratory conditions, congestive heart failure or other conditions that have weakened their physical health. Although it varies by individual, most patients receiving skilled nursing care are discharged to their place of residence within 30 days.
Our dedicated team provides them with round-the-clock care, including:
- Nursing care
- Rehabilitation therapy, including speech/language, physical and occupational therapies
- Physician services
- Rehabilitative nursing to help with the transition back to daily life at home
- Nutritional care
- Group social activities
- Spiritual support
The team includes registered nurses (RNs), licensed practical nurses (LPNs), certified nursing assistants (CNAs), and physical, occupational and speech therapists. They are supported by a full-time administrator, director of nursing, assessment coordinator, case manager, social worker, activities director and registered dietitian. A physician specially trained in wound care is also available as needed and spiritual caregivers visit regularly to serve residents.
Medicare covers the first 20 days of skilled nursing care. After that, it will pay part of the stay and the resident and/or supplemental insurances pay the co-payment as long as the resident meets Medicare skill guidelines. Once skilled care is no longer required or Medicare benefits are exhausted, our social worker and business office staff will work with the resident and the family to determine available payment resources.
