The Conemaugh Health System (CHS) completed a Community Health Needs Assessment (CHNA) for the hospitals of CHS which serve the same primary service area -- Cambria and Somerset Counties.
The CHNA identifies challenges and issues that may negatively impact the health of residents within the two county area. With the findings, each hospital developed a separate implementation plan to remediate identified issues. Action steps of each implementation plan will be monitored for completion against performance metrics and will be updated annually.
Hospitals within a system can jointly prepare a CHNA as long as they serve the same service area. Each hospital must develop a separate implementation plan to be attached to each hospital's 990.
